]]> By: Sue Ann Koop http://www.arizonaseniorlaw.com/blog/2009/10/automatic-points-for-the-medical-pas/comment-page-1/#comment-8382 Sue Ann Koop Sun, 22 Nov 2009 02:04:59 +0000 http://www.arizonaseniorlaw.com/blog/?p=192#comment-8382 Dear Jill Preston, thank you for helping me but you did say to call you abck if i ran into any rouble obtaining servicesi went on line immediately and did what you said.....but nothing happened and when i called the altec social service they could find my name but nothing else that i filled out, then they said i could not fo the questions on the computer or on the phone and they never received my application that i mailed notrmal mail either they think may have been filed uder coop with a c not a k,they treansferred me to 4 departments then sent me to a extention which was a lawyer how frustrating my BP went up to 100 over 164- and i am usually 90> over 60-- i was getting frustrated so my friend helped me with the call i was crying they sent me to another lawyer- i explained i had already done that part with you over the phone and had added the info directly after the phone call onto the computer and mailed in a completed form too!and it was sent there certified mail my friend had them send me another paper for with just name affress etc said i needed to start process again You spent a lot of time with me over 3-4 phone calls, i had all the information ready with medical recoeds finances and finance data. You told me i passed the medical PAS and did meet the income and resource criteria for ALTECS. i did exactually as you said went right away on computer printed copy of form but no one called for 3 weeks my frinds were having to be here most of the time because i was stressed and less stable emotionaly and psychologically as wel as physically. when i get stess ed i use my nebulizer and inhaler more often causing me further instability, i have allrgies food, inhalent and multiple chemical sensitivitis i took shots 3-5 times a week for 20 years now i have my inhaler and order for home oxygen and a porceline mask that the doctor ordered for me. I have been diagnosed with MS exacerbating then remittingafter my first stroke by MRI MRA and neurology department I have had 2 acute strokes in the cerebeller *R and L side )and another stroke frontal lobe which also caused scarring and i have been diagnosed with seizures will be on tegretal lifelong too! in addition to unstable INR coagulation factors i bleed tarry school andnose bleeds then have no evidence with the goag x machine that i am even on coumadincannot turn to the left without fallingmy left arem can be paralyzed with minor exertion due to circulation issues diabetes PAD and neuropathy at times many times i am unable to stand or feel my left lower leg and toes purple toes and fingers as well leg is slidding mpre often with less muxcle control and strenght to get up off the floor when i fall insuranc human has ordered nurseing and PT to set up programs for a medical assistant to continue after their visists are up resp therapist to set up oxygen equipment i have a nebulizer and they will make sure i have the saline and atavert to use the nebulizer equipment andthey will teach the CNA or companions because they will always be the ones taking care of me in thid job to get me to have the highest possible quality of life i am crying as i write this this please help me the humana did not even cocordinate the home care although the approved me as eligible and said i did not need a referral i was PFFS and could dhoose any of the home care agencies i canot d that alone so sonja is going to come and spend the entore day trying to set this up Jil i will be more than happy to send you a fe you are the one that has helped me the most and i will send a check for your services please help me-i want a better quality of life and i am feelling that no matter how hard i try to get the services i need i can still make something go wrong-please tell me your fe i wil get in witer in the mail tomorrow or ask my husband to call you willth his credit card okay thankj thank you ythank you my humana nurse that comes here and told me abut you is marrian alessorio i have her card and her bosses card hwhose name is michaela west they will help you with anything you need or to coordinate please please help i have gone downhill just trying to wait gor this to get straight and i am crying a lot hopeless feeling my number is 6026265752 you can leave a message i cant get to the phone anymore my care helper det a cell phone up but i need to look up he number thank you again sue ann koop 6340 S 20th Glen Phoenix, AZ 85041 one of the helpful neighborhood children is tyoing this with me i am so fortunate my neighbors children help me a lot with tech things thank you a millio teach nursing assistants about that as well and rn to make sure i get all my medications delivered to the door and is to teach and oversee the Nure aids sent from altedsoh assistancts (the assistants 36 hours a week will call in to nurse and the coag machine and diabetic montior witll call tapestry to report to the doctor like sonja and yesuko i fall from 2 to 10 times a day on the average, because of the falls several caused severe inability to move without spasms and needed to transfer to a potty chair unable to walk have broken 3 shower chairs and also 2 walkers, use a cane and go slowly but still fall very frequently I have a nebulizer with a botle of ataxert medicine an dnotrmal aline, also i have a prescription for at home oxygen usefro 2 years was using 7 liters for 3 hours at doctors office now can use 3 ml at home. multiople falls fro bathroom shower and many cuts from falling and sliding with blood large amounts didfficult to clot because am on lfetme coumadidn for 3 three coagulation disorders Protein c and s and antithrombin also SLE lupus cause clottin too!
